Even though I have not seen everything, I’m getting close. And this is not only way too funny, it is another example of things you really can’t just make up.

From what I've read, this is set up so anyone can insert any sort of dialogue that they want. Like the Hilter rants, it's generic and limited only by the users imagination.
Which in this case is based on someone being in medicine of some sort for a long time.
